Abstract

The interRAI Intellectual Disability Collaborative Action Plans (interRAI ID CAPs) were developed to promote social and community engagement among persons with ID. They identify individuals who may benefit from additional supports or services in specific areas and are designed to inform planning. They can be used in conjunction with other interRAI assessments when the person is also receiving formal health services (such as home care, chronic care, long-term care, and mental health care). The ID CAPs can be used in both community-based (for example, own home, semi-independent living, group home, family home) and institutional settings.The ID CAPs are not diagnostic tools aimed at labeling the individual or identifying areas in which he or she should be “cured.” Rather, the CAPs identify a range of issues in daily life that may be addressed to enhance social and community engagement. The interRAI ID Assessment System does not treat engagement as an “all or nothing” outcome. There are differences in the degree and type of engagement that each person prefers. Still, the underlying assumption is that engagement is possible for all persons with ID.This book includes introductory material followed by the CAPs themselves.
