Abstract

interRAI Child and Youth Mental Health Collaborative Action Plans: For Use with the Child and Youth Mental Health Assessment Instrument, known as CAPs, are designed to incorporate evidence-informed practice and suggest recommended interventions, accompanying multi-disciplinary mental health assessment of children and youth between the ages of 4 and 18 years. CAPs assist the assessor in systematically interpreting the information recorded on the ChYMH Assessment Form. CAPs are not intended to automate care planning; rather they help the clinician focus on key issues identified during the assessment process, so that decisions as to whether and how to intervene can be explored with the young person and family.This book includes introductory material followed by the CAPs themselves, which are organized into the five broad areas of Safety, Family Life and Social Integration, Functional Status, Health Promotion, and Services and Support. The appendices provide a summary of interRAI scales and algorithms referred to in the CAPs.
