| Abstract | Discharge planning processes include assessment, documentation and communication. In clinical practice formalised and informal mechanisms facilitate these processes. The role of a standardised comprehensive assessment instrument, the MDS-HC (Minimum Dataset-Home Care), linked with structured documentation, was evaluated as a component of the discharge planning process on geriatric rehabilitation wards using a qualitative methodology. |
