Abstract | This edition of the interRAI Acute Care for Comprehensive Geriatric Assessment (AC-CGA) contains country-specific examples for Australia and uses the English native to Australia. The interRAI AC-CGA is a key element of the interRAI Hospital Assessment Systems. It is designed for use among older (and some younger) patients who have complex functional and psychosocial needs. The interRAI AC-CGA Assessment and Care Planning System is a comprehensive, standardized instrument for evaluating the needs, strengths, and preferences of older people, who are judged to have major functional and psycho-social issues that are likely to affect their management in hospital or discharge planning. The interRAI AC-CGA consists of an assessment form, item-by-item instructions, and a series of care planning protocols. |
