Abstract | Inconsistencies in home and community care data collection across Canada was the driving force for changing the process of assessment of case-managed clients in British Columbia. Clinicians had been using a paper-based assessment tool but were mandated to implement the RAI-HC and to conduct assessments using a computerized version. This led to a significant change in clinical practice. The authors provide an overview of the transition in the Interior Health Authority, linking phases of the process to Kotter's model of creating major change. |
