Abstract | This edition of the "interRAI Community Health (CHA) Assessment Form and User's Manual" is customized to contain country-specific examples for Canada, and it uses the English language that is native to Canada.The CHA instrument was developed in response to user requests for a modular instrument that could efficiently assess the well elderly individual and identify those persons who merit further assessment in order to prevent or stabilize early functional or health decline. The instrument comprises a brief basic assessment and four supplements: Functional (CHA-FS), Mental Health (CHA-MH), Assisted Living (CHA-AL), and Deafblind (CHA-Db). The target population is adults living in the community and a variety of residential care settings.This book contains introductory material; an item-by-item guide to the basic CHA assessment form; triggers and algorithms for the four supplements; item-by-item guides for each of the four supplemental assessment forms; the basic assessment form; and all four supplemental assessment forms. |
