Abstract | The CMS coRAI pilot projectConducted in 2017 in 6 health centers (CMS) of the canton, this project aimed to optimize the choice of the geriatric assessment tool according to the client's profile of help and home care. He tested two new tools: an interRAI Emergency Department Screener (iEDscreener ©) and a "short" geriatric assessment instrument (interRAI Contact Assessment Test Version (iCAtest)) lighter than the RAI -Home Care Switzerland (RAI-HCSuisse) usually used in home care clients. Among the objectives were to test the use of a screening tool by Nurse Analysts (IA), specialized in the analysis of requests for help and home care, as well as to test the iCAtest evaluation. by Nurses Referring Situation (IR). Finally, the pilot sought to determine the optimal articulation between the iEDscreener, iCAtest and RAI-HCSuisse.ProcessThe iEDscreener was administered by phone by RNs to all new clients. In a few questions, this tool makes it possible to define a risk score of adverse health events according to the interRAI Assessment Urgency Algorithm (iAUA). The higher this score (values: 1 to 6), the higher the risk.The iCAtest was administered by RIs to all clients seeking practical help only (nonOPAS7) and RAI (IE) nurse evaluators to clients seeking care (OPAS7). For the latter, the iCAtest was used when the client had an iAUA score of less than 4, while the RAI-HCSuisse was used if the iAUA score was equal to or greater than 4 ("at risk" profile). |
